Virtual Wards are a digital solution for clinicians to support patients in their homes. It is a ‘wrap-around’ service that ensures patients can quickly receive the care they need whilst also reducing the need for avoidable hospital admission.

What is the aim of Virtual Wards?
We offer a holistic service to support the NHS in monitoring patients using a Virtual Ward.
Clinicians refer their patients to the Virtual Ward to help the patient monitor their symptoms at home.
How do Virtual Wards work?
Our Long-Term Conditions Team registers the patient with the Virtual Ward.
Instructions are emailed to the patient explaining how to access our service, including a link to our system to monitor themselves at home.
The patient is asked daily question sets and is provided with any peripherals needed to take their daily readings. The easy-to-use online clinical portal allows clinicians to quickly see if the patient is deteriorating to act when required.
We have extensive experience offering Virtual Ward solutions to the NHS, including our highly regarded, award-winning COVID Virtual Ward provided to patients in South West England. This service has supported over 7,500 patients to date. Patients on this service have been provided with a pulse oximeter and are asked to enter readings and answer wellbeing questions daily.
